| Evergreen Health and Rehabilitation Conveniently located in the city of Winchester, Evergreen Health and Rehab Center offers exceptional skilled nursing services to enhance the resident’s abilities and independence. Short and long-term services are provided by the centers nursing and rehabilitative staff. Our center is located on beautiful grounds in a residential neighborhood helping us meet our goal of being your community rehab center of choice. About Us Like hospitals nursing centers never close. Services must be available 24 hours a day, 365 days a year with trained, licensed nursing staff always present. A nursing center is required to maintain interdisciplinary staffing at several levels, including a licensed nursing facility administrator, physician medical director, director of nursing services, nurses trained to provide skilled nursing care, a social worker, activity director, and director of dining services. Evergreen Health and Rehab also employs or contracts with a physical, occupational and speech therapy, and a registered dietician. Evergreen Health and Rehab is licensed under state law, and is an approved Medicare and Meidcaid provider. As such we are required to meet strict Federal and State Certification standards on quality of care and of life and resident’s right. A variety of social events and activities are designed to add richness t the leisure time of our residents. They are encouraged to continue prior hobbies, make new friends, join our “Lunch Brunch” for outings or participate in any of a number of group activities offered daily. Several local denominations provide religious services and bible studies to help meet the spiritual needs of our residents. Family participation is encouraged from day one. No one knows the resident as well as the family. Your input will help us better meet the resident’s needs. Back to Main Page Scope of Services Rehabilitation Services- Evergreen Health and Rehab provides comprehensive physical, occupational, and speech therapy programs. Individualized treatment plans are developed by our therapist for each resident. Our therapy training includes strengthening and reconditioning treatment for post-stroke or surgical conditions, prosthetic training, retraining in daily living and home living skills, splinting, and retraining in swallowing, speech and communication skills. Back to Main Page Skilled Nursing Services Other skilled services may include post surgical care, stroke recovery, IV therapy, oxygen therapy, wound management, and tube feeding services. An interdisciplinary team comes together to develop a resident medical plan of care to help each resident achieve their optimum level of social and emotional functioning. Back to Main Page Intermediate Care Services These services are provided to those who require daily monitoring and medication management of chronic conditions that cannot be managed in the home setting. Services include assistance with bathing, dressing, eating, and incontinent care. Nursing restorative care may also include bowel and bladder retraining and ambulation training and assistance.
